PARIS (AP) ? It's a July evening on the terrace of the legendary Cafe Flore. A coiffed woman sips chilled wine, another savors her chocolate eclair.
The one thing to complete a perfect picture of Parisian life? A dash of French rudeness.
It comes from the waiter, who snootily turns away a group of tourists: "There's no point waiting," he shrugs, even though there are many empty tables. "No space outside."
Such rituals of rudeness have long been accepted by visitors as part of the price of enjoying such a beautiful city as Paris. But it seems the French themselves, who over centuries have turned rudeness into an art form, have become fed up with their own incivility, according to recent polls and publicity campaigns.
There's a fabled history of French rudeness from Napoleon, who called the English a "nation of shopkeepers," to former President Nicolas Sarkozy, who infamously snarled at a voter: "Get lost, poor jerk."
Now, bad manners and aggressive behavior top the list of causes of stress for the French, even higher than unemployment or the debt crisis, says pollster IPSOS. A total of 60 percent cited rudeness as their number one source of stress in a survey last year on social trends.
"We're so rude," admits 34-year-old French teacher Stephane Gomez, as he comes out of a Paris metro station. "France lacks the civic sense that you find in Anglo-Saxon countries."
"It's so easy to be polite, but we don't do it," says 30-year-old Zahia Sebahi. "I never see someone give up their seat for an elderly person."
But Paris's public transport authority is leading the fight-back in a summer-long publicity campaign against rudeness.
Billboards depicting Frenchmen with animal heads have dominated metro stations; they target passengers who are rude to staff and push and shove. "If you shove five people getting onboard," the posters say, "it won't make us set off faster." Bus ads read: "One bonjour doesn't cost a penny, and it changes your day."
Transport officials say they've tried to keep a light touch on a serious matter.
"We used humor to not be moralistic," says Isabelle Ockrent, RATP communications director. "But we've been alerted by our staff that there is a real problem."
Public transport staff even held "rudeness forums" over three days in late June in 20 metro stations, in which they exchanged views on correct etiquette with passengers.
Among other things, people were asked what they thought the root causes were for pushing and shoving: Lack of time? Unhappiness? Many Parisians attribute the decreasing tolerance to rudeness to the fast-pace of the Internet age, with i-Pods on the metro and mobile phone noise in public spaces.
"It might seem obvious, but when stressed for work, Parisians forget 'hello' or 'thank you' when asking for a ticket," says Ockrent.
Parisians, it seems, may be pleased to be going back to finishing school.
Passengers regularly gawk at the ads, which have stretched from ceiling to floor inside some metro stations, and the RATP say the campaign has gotten near universal positive feedback on it.
The results of this year's IPSOS trend research won't be published until fall. But the agency can already reveal that the backlash against rudeness is rising.
"In 2012, one thing is clear; the French are irritated and want a return to good manners. They've had enough of rudeness ... reached a limit," says Lise Brunet, IPSOS's director of trend studies.
"The acceleration of life with cells phones means that people have even less time to follow the rules of politeness," adds Brunet. "Today, it's even more of a concern than the economic crisis. People just want to hear 'Thank you.'"
In France, the world's most visited country, rudeness is also a concern for tourism companies ? especially as France feels the bite of the financial crisis.
Atout France, the country's tourism development agency launched a summer campaign in regions where tourism is suffering to promote service quality.
"We're very aware of the problem making tourists feel welcome," says Fanny Moutel, communications director for Atout France, the country's tourism development agency. "We've noticed that there are fewer English visiting places like Brittany and Normandy and the Loire, so the campaign aims to improve the way tourists are treated."
Where English-speaking tourists were once greeted with raised eyebrows and a Gallic shrug, more and more French in the service industry speak English.
That's just one of the many ways in which France is changing.
Earlier this week on a high-speed train, there were puzzled smiles from passengers after a decidedly un-French loudspeaker address:

A Surprising Result
Little worked on this algorithm while he was getting his PhD at Oxford. It didn't occur to him that it could be used to detect Parkinson's disease until a chance encounter with a researcher from Intel.

Voice 2 from the audio above demonstrates symptoms of Parkinson's disease.
"He came up to me and said, 'Max, we've got vast amounts of data recorded from Parkinson's disease patients and we really don't know what to do with it. So perhaps your algorithms might be useful for analyzing this,'" Little remembers.
The recordings were voices of people both with and without Parkinson's and Little's algorithm sorted them. It was 86 percent accurate.
"And that's when we thought 'Ahh, okay, we got something here,'" Little says.
Gathering Voices
Now, he's looking to crowdsource more voice recordings of people with and without Parkinson's to make the algorithm even more accurate.
Last month, Little and his team asked people all over the world to call up their phone banks and record their voices. The caller follows a series of prompts ? asking them to say "ahh" for as long as they can and say a few sentences.
"We expected to it to run for six months and collect 10,000 voice recordings over that period," Little says. "In just under a month, we got 5,000 voice recordings."
The algorithm listens for three main clusters of symptoms in the voice: vocal fold tremors, breathiness and weakness, and the way the jaw, tongue and lips fluctuate during speech.
Little hopes the algorithm will improve enough so that eventually a person could get a diagnosis and track their illness through the phone.
He admits it's still in the early stages, but it could be the simplest way to diagnosis Parkinson's ? and a way to help patients get earlier treatment.
"It's a very tantalizing possibility. If voice is one of the early indicators of the disease, then we have a very cheap way for looking for those who are susceptible in the population," Little says.

AURORA, Colo. (AP) ? A U.S. Navy veteran who served three tours of duty in the Middle East. A 6-year-old girl excited about her swimming classes. A Target employee who shielded his girlfriend and her brother with his own body. They and nine others were killed in the shooting rampage during a midnight showing of "The Dark Knight Rises" in a Denver suburb. Here are their stories:

Jonathan Blunk had high hopes for the future, with plans to re-enlist in the Navy and the goal of becoming a Navy SEAL.
The 26-year-old served three tours in the Persian Gulf and North Arabian Sea between 2004 and 2009, said close friend James Gill of Brighton, Colo.
"It was guts or glory for him," Gill told The Associated Press. "It always surprised me that he didn't serve in a situation more on the front line. He wanted to be a first responder on the front line."
Blunk was also a certified firefighter and emergency medical technician, Gill added.
He died in the shooting Friday after throwing himself in front of friend Jansen Young and saving her life, she told the Today Show. He told her to stay down.
"That's something he would do," Gill said. "If he was going to choose a way to die, that's how he wanted to go ? defending someone from a (person) like that."
Blunk, a 2004 graduate of Reno's Hug High School in Nevada, most recently worked at a hardware store.
His estranged wife, Chantel Blunk, lives with their 4-year-old daughter and 2-year-old son in Sparks, Nev.

Alexander J. Boik, known as AJ, recently graduated from high school and was to start classes at the Rocky Mountain College of Art and Design in the fall, The Denver Post reported.
The family said in a statement that the 18-year-old was loved by all who knew him and was dating "a beautiful young lady" who was with him at the theater and survived. "We want to try and focus on the beautiful lives that were ended and not the evil that is responsible," the family said.
A friend, Jordan Crofter, described Boik as someone who "didn't hold anything back. He was just his own person."
"He was a ball of joy. He was never sad or depressed. He wanted everybody to be happy," Crofter told The Associated Press.
Crofter said Boik played baseball from when he was a child through his junior year in high school.
He said Boik and his girlfriend were the "perfect couple" and people expected them to get married.
"If he were still here, he'd try to make everyone have a positive outlook of the situation and not allow it to affect their outlook of life," Crofter said.

Jesse Childress was an Air Force cyber-systems operator based at Buckley Air Force Base, Colo.
Air Force Capt. Andrew Williams described the 29-year-old from Thornton, Colo., as knowledgeable, experienced and respectful. "We're going to miss him incredibly," he said.
Tech Sgt. Alejandro Sanchez, a co-worker, told the AP that Childress was his good friend and they were on a bowling team together.
"He would help anyone and always was great for our Air Force unit," he said.
Another co-worker, Ashley Wassinger, said Childress "was a great person fun to be with, always positive and laughing."
"Really just an amazing person, and I am so lucky to have been his friend," she said.

Gordon Cowden loved life and his family, and he had gone to the midnight movie premiere with his two teenage children.
At 51, he was the oldest of the victims killed in the shooting. He lived in Aurora, but was described as a "true Texas gentleman" in a family statement. He loved the outdoors and owned his own business.
"A quick witted world traveler with a keen sense of humor, he will be remembered for his devotion to his children and for always trying his best to do the right thing, no matter the obstacle," his family said.
His teenage children escaped the shooting unharmed.
His family declined to be interviewed in their request for privacy, but expressed appreciation for words of concern offered in the wake of the shooting.
"Our hearts go out to everyone that has been harmed by this senseless tragedy," they said.

Jessica Ghawi recently wrote a blog post after surviving a shooting at a Toronto mall, saying it showed her "how fragile life was."
Friends say the 24-year-old, who moved to Colorado from Texas about a year ago, didn't let the June 2 shooting in Toronto change her outlook on life as she pursued a career in sports journalism.
"I think she even looked at that like, 'Hey, even after that, I'm able to pursue my dream," said Peter Burns, a radio sports show host with Mile High Sports Radio in Denver, where Ghawi recently interned.
That shooting left two dead and several injured. Her blog post last month said: "I saw the victims of a senseless crime. I saw lives change.
"I was reminded that we don't know when or where our time on Earth will end. When or where we will breathe our last breath."
Former colleagues described her as ambitious and hardworking. She went by the name "Redfield," a play on her red hair, because it was easy to say and remember, both professionally and on her social media accounts.
She was a regular tweeter and her last post to the micro-blogging website stated in all capital letters, "movie doesn't start for 20 minutes."

John Larimer was a Navy sailor based at Buckley Air Force Base, where he was a cryptologic technician ? a job that the Navy says on its website should be filled by someone with "exceptionally good character, above-average writing and speaking skills, a good memory, curiosity and resourcefulness."

The 27-year-old and another active service member, Air Force Sgt. Jesse Childress, were killed in the shooting rampage, the military said Saturday.
Larimer, who grew up in the Chicago suburb of Crystal Lake, joined the service just over a year ago, the Navy said.
"A valued member of our Navy team, he will be missed by all who knew him. My heart goes out to John's family, friends and loved ones, as well as to all the victims of this horrible tragedy," said Cmdr. Jeffrey Jakuboski, his commanding officer, in a written statement.
A family member told the Daily Herald newspaper in Arlington Heights, Ill., that Larimer was the youngest of five siblings. Neighbors in his hometown recalled his sense of humor.
"We love you, John, and we will miss you always," his parents said in a statement.

As the attack in the movie theater unfolded, Matt McQuinn dove in front of his girlfriend and her older brother to shield them from the gunfire.
He died protecting them, said Rob Scott, an Ohio attorney retained by the families of McQuinn and his girlfriend, Samantha Yowler. Scott confirmed McQuinn's death to The Associated Press on Saturday. He was 27.
"Unfortunately, Matt McQuinn perished from the injuries he sustained during the tragic events that unfolded in Denver, Colorado, and went home to be with his maker," Scott said in a statement. "As both families mourn the loss of Matt, they ask for everyone to give them distance and time. Again, the families thank everyone for their love, prayers and ask that we respect their families' wishes."
Yowler was recovering from surgery after she was shot in the knee at the theater. Her 32-year-old brother, Nick Yowler, who also shielded his sister, was not injured.
McQuinn and Yowler moved to Colorado from Ohio last fall. A Colorado co-worker told the Springfield News-Sun that McQuinn and Samantha Yowler worked with her at Target.
"They're really fun people. We always go out together," said Melissa Downen.

The death of 23-year-old Micayla Medek was heartbreaking, said her father's cousin, Anita Busch.
But Busch said the news also was a relief for the family after an agonizing day of waiting.
"I hope this evil act ... doesn't shake people's faith in God," she said.
Micayla Medek lived in the Denver suburb of Westminster and attended Aurora Community College.
Her aunt, Jenny Zakovich, 57, of South Milwaukee, Wis., said Medek and her father were both huge Green Bay Packers fans.

The youngest of the victims killed in the attack was Veronica Moser-Sullivan.
She had just learned to swim, and at age 6, she was a "great little girl, excited about life," her great-aunt Annie Dalton said. "She should be at 6 years old."
Her mother, Ashley Moser, remains hospitalized in critical condition with gunshot wounds to her neck and abdomen. She has been in and out of consciousness and asking for her daughter during moments of lucidity.
"Nobody can tell her about it," Dalton said. "She is in critical condition, but all she's asking about is her daughter."

Alex Sullivan's family called him "their real life super hero," and he was at "The Dark Knight Rises" premiere celebrating his 27th birthday and his first wedding anniversary.
"Alex was a gentle giant, known and loved by so many. He always had a glowing smile on his face and he made friends with everyone. Alex enjoyed all sorts of movies, was an avid comic book geek and loved the New York Mets," the family said in a statement.
Sullivan had a warm smile and an innocence that endeared him to people, said Shelly Fradkin, whose son Brian was good friends with Sullivan.
She sat next to a makeshift memorial Friday near the theater where an oversized birthday card with a photo of a smiling Sullivan was displayed.
"He's amazing. He was just a big teddy bear. Great hugs," she said.
She said Sullivan was such a big movie fan that he took jobs at theaters just to see movies.
Fradkin and her son spent an "excruciating" day trying to find Sullivan before learning of his death, she said.
"We're shocked. We're numb. We're sick," she said. "Our hearts are broken, and we're crushed."

Alexander C. Teves, 24, of Phoenix, earned a master's degree in counseling psychology in June from University of Denver.
He was a lovable person who made friends quickly and had a lot of them, said his grandfather, Carlo Iacovelli of Barnegat, N.J.
As a boy, Teves moved from New Jersey to Phoenix with his parents. Iacovelli and his wife wintered there and spent a lot of time with him.
"He was what you might call an ideal grandson," Iacovelli said. "He was a fun guy. He loved to eat."
Teves was planning to become a psychiatrist, his grandfather said.
"He had a lot to look forward to," Iacovelli said.

Rebecca Ann Wingo had started a job several months ago as a customer relations representative at a mobile medical imaging company. She was 32.
Shannon Dominguez, who worked with Wingo on weekends, said she was friendly with everyone and always seemed to be in a good mood.
"I didn't really know her well but she had a really bubbly personality," Dominguez said. "She was a pretty happy person. She just never really seemed ... like with work, she never got irritated. She was pretty happy to be here."

A growing number of employers are giving workers paid vacation time these days. The only thing is, many of you don?t understand what vacation is all about.
Vacation means taking time away from work, relaxing and recharging. That means, not working.
Unfortunately, more than half of U.S. workers plan on working during their vacations this year including everything from checking emails to doing actual work tasks.
A poll released this week by software company TeamViewer and conducted by Harris Interactive in May, found that 52 percent of those surveyed will be working while on vacation, up from 46 percent the previous year.
Here?s how the workaholic?s vacation/work schedule breaks down, according to the survey:
- Reading work-related emails ? 30 percent
- Receiving work-related phone calls ? 23 percent
- Wanting access to a document on my home computer ? 19 percent
- Receive work-related text messages ? 18 percent
- Wanting access to a document on my work computer ? 13 percent
- Being asked to do work by a boss, client or colleague ? 13 percent.
The worst gender for this vacation offense are men with 56 percent saying they were more likely to work, compared to 47 percent among women.
And the one group that can?t seem to get a break is single working Americans, who expect to be asked to do work by the boss more often than their married counterparts, 15 percent versus 6 percent.??
For many workers, the decision to keep working through R&R times, is about making sure jobs are secure and going above what?s expected in order to impress employers.
But that can be a recipe for disaster and may ultimately hurt your job performance.
"Rest and renewal ultimately increase our ability to be productive, it is essential to completely unplug when on vacation," said?Susan Steinbrecher, a business consultant and author of "KENSHO: A Modern Awakening, Instigating Change in an Era of Global Renewal."
"Most people don?t take renewal seriously," she continued. "I believe our connected, always on, 24/7 society has lost the ability to recharge and renew without distractions. The minute you check an email or voice message while on holiday, you?re likely to get sucked right back in."
Some employers seem to realize the importance of vacation for their workers. The number of employers offering vacation benefits is actually on the rise.
Today, more than half of organizations now provide paid time off, including vacation days, compared to 42 percent in 2009, according to a report released in June by the Society for Human Resource Management.
And some firms are actually trying out unlimited vacation policies.
Alas, many workers still aren?t taking their vacation days seriously, or should I say, un-seriously.
"Today?s work environment of intense time pressures and limited resources means we are all required to put in extra effort, energy and time ? which can create a lot of stress," Steinbrecher maintained. "This 'do more with less' work ethic means that if we don?t completely disengage when on vacation, we?re not fully recharging or refueling."

Do people change? Yes, and if you respect each other, you will grow together in ways that will allow you to complement each other and make you stronger together than you are apart.
Look, I know this is hard, but the reward for this honesty and commitment is a lifetime relationship with someone who really cares.? Isn?t that worth the effort? Isn?t that what life is really all about?

Parental Abuse Increases Child's Risk of Cancer in Adulthood ...

Purdue University researchers discovered the risks are significant when mothers abuse their daughters and fathers abuse their sons.
?People often say that children are resilient and they?ll bounce back, but we found that there are events that can have long-term consequences on adult health,? said Dr. Kenneth Ferraro, distinguished professor of sociology.
?In this case, people who were frequently emotionally or physically abused by their parents were more likely to have cancer in adulthood, and the link was greater when fathers abused sons and mothers abused daughters. Overall, the more frequent and intense the abuse, the more it elevated the cancer risk.
?We would like to see child abuse noted as an environmental factor that can increase cancer occurrence in adulthood. More research on this topic also could help mediate the effects or improve interventions to help abused children.?
The research is published online by the Journal of Aging and Health.
?We started examining a variety of childhood misfortunes, including abuse, and when these were all combined, we found that men with the most stressors during childhood were more likely to develop cancer,? said co-author and sociology and gerontology graduate student Patricia Morton.
?Second, we found that when children were abused by their same-sex parent, it increased their cancer risk,? possibly becaue of the greater social bond between same-sex children and parents.
?Other studies have shown that if a mother smokes, the daughter is more likely to smoke, and the same relationship is found when sons mirror their father?s behavior,? Morton said.
?More research is needed, but another possibility is that men may be more likely to physically abuse their sons, and mothers are more likely to physically abuse their daughters.?
Researchers reviewed survey data from 2,101 adults in two waves of the National Survey of Midlife Development in the United States.
Abuse was one of many childhood misfortunes ? including poverty, loss of parent and family educational status ? that researchers examined to determine if there was a link to cancer in adulthood.
One method of analysis looked at cumulative stress during childhood. This review found that men who experienced the most cumulative stressors during childhood were more likely to have cancer. This was not true with women, and this suggests that men and women may have different responses to childhood stressors, Morton said.
A second perspective looked at each category of misfortune ? a viewpoint that identified the connection between abuse and cancer. In this analysis, survey participants were not directly asked if they were abused, but abuse was defined by survey answers to questions such as how frequently a parent, sibling or other person insulted or swore at them as a child; refused to talk them; threatened to hit them; pushed, grabbed or shoved; threw something at them; kicked, bit or hit them with a fist; choked them; or burned or scalded them.
The frequency of these abuses also was identified. Researcher then controlled for factors such as an adults age, lifestyle choices and economic status yet the association between abuse and cancer remained.
?It also is likely,? Ferraro said, ?that the effect between child abuse and cancer is underrepresented in our study, because people who suffered abuse and were then incarcerated, placed in a mental institution or died were not included in this survey of adults. These groups may represent people with more acute and severe effects from abuse, and even though they are omitted, we still find a link.?
As a result of the findings, researchers are expanding their investigation of the effects of child abuse and health conditions in adulthood. Scientists are now examining potential links between child abuse and other health outcomes, including heart attacks and types of cancer.
?The connection between negative childhood events and mental health is accepted, and these findings reinforce that such events can also have a long-lasting effect on a person?s physical health,? Morton said.
?It?s shocking just how much the damage sticks, and it is a reminder that childhood, which is defined by rapidly changing biological systems, is a sensitive period of development.?

Senate panel approves trade relations with Russia
WASHINGTON (AP) ? A Russia trade bill that could double U.S. exports to the former Communist superpower but complicate already frosty relations with Russia advanced in the Senate Wednesday.
The Senate Finance Committee combined the trade measure with a bill to punish Russian human rights violators. But lawmakers rejected a provision that would have required the president to certify that Russia is no longer supplying arms to Syria.
The committee's unanimous vote to lift Cold War trade restrictions and establish permanent normal trade relations with Russia came against a background of strong objections to Russia's poor human rights record, its threats against U.S. missile defenses in Europe, its failure to protect intellectual property rights, its discrimination against U.S. agricultural products and most recently its support for the Assad government in Syria.
Enacting permanent trade status is necessary if U.S. businesses are to benefit from the lowering of trade barriers that will take place when Russia enters the World Trade Organization next month.
Finance Committee Chairman Max Baucus, D-Mont., said current U.S. exports to Russia, about $9 billion a year, could double in five years if trade relations are normalized.
"There is no time to waste. America risks being left behind," Baucus said. "If we miss that deadline, American farmers, ranchers, workers and businesses will lose out to the other 154 members of the WTO that already have PNTR (permanent normal trade relations) with Russia." U.S. imports from Russia last year were four times the export level.
Getting the trade bill through Congress has been a top priority for business and farm groups, which see it as a jobs creator and a boost to the economy. "Without PNTR, U.S. companies and workers will be at a distinct disadvantage in the Russian market as our competitors in Europe, Asia and elsewhere begin to lock in sales and long-term contracts," said Caterpillar Inc. chairman and CEO Doug Oberhelman, who also chairs the Business Roundtable.
But it has struggled to gain momentum because of poor relations between the two countries.
"It is with some trepidation that we undertake this task," said Sen. Orrin Hatch of Utah, top Republican on the committee. "Despite President Obama's reset policy, U.S.-Russian relations remain rocky at best."
The bill only advanced to a committee vote after Baucus agreed to link it to legislation, pushed by Sens. Ben Cardin, D-Md., and Jon Kyl, R-Ariz., that imposes sanctions such as visa denials and asset freezes on Russian government officials involved in human rights violations. The legislation, which has sparked strong criticism from Russian officials, specifically targets those involved in the case of Sergei Magnitsky, a lawyer who died in a Russian jail in 2009 after allegedly being subject to torture.
Sen. John Cornyn, R-Texas, proposed an amendment stating that the legislation would not go into effect until the president certifies that Russia is no longer supplying arms to Syria. "If it's important to vindicate the rights of a single person," he said, referring to Magnitsky, "how much more compelling is it" to ensure that Russian arms aren't being used to kill thousands of Syrian civilians. It was defeated, with opponents saying withholding permanent trade status would only hurt U.S. businesses, not Russia.
How to proceed with the trade issue has divided both parties. Last month Hatch and seven other Finance Committee members sent Baucus a letter listing the many troubling aspects of U.S.-Russia relations and questioning whether Russia would comply with WTO findings when disputes arise. But earlier this week 73 House Republican freshmen wrote President Barack Obama saying that "we will only hurt ourselves and lose out on economic opportunities and needed jobs" if full trade relations with Russia are not established.
It's a "Hobson's choice," said Sen. Mike Crapo, R-Idaho, at Wednesday's committee meeting. By moving to help U.S. exporters Congress was put in the situation of rewarding the bad behavior of the Moscow government, he said.
While adding the Magnitsky provision to the bill, the measure eliminates another human rights act that has been the main stumbling block to permanent normal trade relations.
The bill repeals the 1974 Jackson-Vanik act that tied trade with the then-Soviet Union to Moscow's allowing Jews and other minorities to leave the country. While Jackson-Vanik has long outlived its purpose, it has remained on the books, a mark of the continued difficult relations between the two countries.
It appears unlikely that Congress can act on the trade bill before Russia formally enters the WTO. Congress has only two more weeks before it takes off for its August recess, time likely to be taken up by election-related tax and spending bills.
On the House side, Ways and Means Committee Chairman Dave Camp, R-Mich., welcomed the Senate's action and said he intended to introduce a bill in the next few days. He said he was working with the White House to find a Democratic co-sponsor so the bill could move through his committee on a bipartisan basis.
The bill also establishes permanent normal trade relations with Moldova. Baucus said the former Soviet republic, which joined the WTO in 2001, is the only WTO member that does not have permanent trade status with the United States.

Study Compares Gene Expression Profiling Tests for Breast Cancer
Newswise ? New research from University of the Sciences? Mayes College of Healthcare Business and Policy suggests that MammaPrint ? a gene expression profiling (GEP) test used to identify whether women with early-stage breast cancer would benefit from chemotherapy ? is the more cost-effective and clinically-useful test than the most frequently used GEP in practice in the United States today. The findings have strong implications for health policy makers and health insurance reimbursement decisions, as well as, for health providers and patient outcomes.
To date, Oncotype DX ? a 21-gene profile assay ? is the most commonly used GEP in clinical practice in the United States. Oncotype DX produces a numerical recurrence score, which places patients in three categories: high risk, intermediate risk and low risk. MammaPrint ? a 70-gene profile microarray evaluation used more frequently in Europe ? reports results via two categories: high risk or low risk for recurrence.
The research of Amalia M. Issa, PhD, MPH, chair of the Department of Health Policy and Public Health and director of the Program in Personalized Medicine and Targeted Therapeutics, which was published online by Cancer in February 2012, compared Oncotype DX and MammaPrint from a third-party payer?s perspective. A 10-year Markov model, which represents changing health states over time, was developed to evaluate the costs and quality-adjusted life years associated with using the two most popular GEP-guided treatment strategies in a hypothetical cohort of 1,000 women with early stage, lymph node-negative, estrogen receptor-positive breast cancer. Outcomes included no recurrence, recurrence, and death. The costs considered included the expenses generated from the administration of the gene test, dispensation of adjuvant chemotherapy and other chemotherapy, as well as cost of treating recurrence, adverse events and end-of-life care.
?Our study is the first to compare two specific gene expression profiling assays head-to-head,? explained Dr. Issa. ?As personalized genomic medicine continues its progress forward, this study demonstrates the value of doing this type of research for society ? to make decisions based on empirical evidence.?
Specifically, the results indicate that patients who received the Oncotype DX test spent $27,882 and gained 7.36 quality of life years, whereas patients who received the MammaPrint spent $21,598 and gained 7.461 quality of life years. This represents a 29% cost-effective advantage, with better quality of life years than the most frequently used GEP test in the United States.
Breast cancer is the second most common malignancy for women in the United States, with more than 200,000 women diagnosed annually. Each day, patients and their health providers must make difficult decisions about treatment and the utilization of adjuvant chemotherapy to reduce the risk of reoccurrence or death. Increasingly, providers are utilizing GEP tests to evaluate prognostic and risk information and determine the best course of treatment.
?New genomic diagnostics and personalized medicine applications are increasingly entering the market, and this broader availability of diagnostics has major implications for health care delivery,? continued Dr. Issa. ?The unexpected findings of this study demonstrate the need for further translational research studies of this type to be conducted before a novel personalized medicine application is implemented widely into practice.?

Kayak?s IPO Moves Forward, Prices Shares At $22-$25
Kayak's perpetually-delayed IPO finally moved forward today, as the company has officially announced it has priced its shares between $22 and $25, and will begin trading on the NASDAQ under the ticker symbol "KYAK." In the most recent SEC filing, the company said it's going to offer 3.5 million shares, in a deal led by Morgan Stanley - Stanley's first since the Facebook IPO.
